How is your data used ?
Your response data is collected anonymously, no information about you is stored with the survey responses.
Data collected through this survey is used to help us inform best practice and improve service delivery.
Responding to this survey will have absolutely no link to the care and treatment that you receive, it will not be used to make any judgements in relationship to your care or treatment.
What data is shared with your health & care team?
Your health & care team will only be able to view the data that they have put into the system:
Your Health & Care Team will input a Contact either Mobile Phone number or e-mail address.
They will be able to View if a Survey request has been Completed or Not.
Anonymous survey responses are then aggregated up and displayed over time to health & care teams.
At no point can the care team view or request an individual response.
