Through conversations with your clinician or health and care professionals, we work together to understand and decide what tests, treatments, management or support packages are most suitable for you, bearing in mind your individual circumstances and preferences.

It brings together the expertise that you have about yourself and what is important to you together with the clinician’s or health and care professional’s knowledge about what is known about the benefits and risks of the available options

BaNES, Swindon and Wiltshire (BSW) are working together over the next five years to progress the involvement of people in their own care and shared decision making. This SDM survey is our first project to test where we are, how we are doing and to help us plan how we can progress in delivering personalised care.
